Be an entrepreneur or company registered in the Czech Republic or abroad
Proof of identity of the natural person - entrepreneur

Are all cash withdrawals from ATMs free of charge?

Unlimited ATM withdrawals do not apply to ATMs subject to an extra fee charged by the ATM operator. Those are usually ATMs operated by non-banking institutions and you will be always explicitly warned of such a fee on the ATM display in advance. You would first have to agree to the fee in order to withdraw cash.
